Requesting Your Medical Records
If you need to request a copy of your medical records, please complete the appropriate form listed below. To process your request, the form must be printed, filled out completely, signed, and submitted to our records department.
How to Submit Your Request
Please follow the steps below:
1. Download the appropriate form from the list below.
2. Print the form.
3. Complete all required fields.
4. Sign the form where indicated.
5. Scan or take a clear photo of the completed form.
6. Email the completed form to: recordrelease@sjhealthnewark.com
